SIRS Home of My Own assists individuals with disabilities to live independently in their own home or remain in their family home. SIRS’ trained team members facilitate developing independent living skills, building community relationships, maintaining health and wellness, and managing the daily activities of home management. Services are customized based on individual needs. Individuals must be eligible for Medicaid Waiver services through the State of Indiana.
